This has been resolved restoring the affected tables to the previous version. Cleared all the cache and all customers restored completely.

Intermittent authentication issues with the DUO Web Integration have now been resolved and all impacted customers restored to a proper state. A cached state could impact some customers, but will be resolved by Saturday evening, US Central time. We are continuing to monitor the situation. Thank you for your patience.
